Campus System Tracking

Dear Colleagues:

We are implementing a new procedure to further protect the health and safety of our campus community as we slowly ramp up research and other on-site activities.

Employees working in campus owned or leased facilities must wear face coverings at all times, maintain 6 feet of physical distance from others and follow hygiene and handwashing guidance.  Employees must not come to campus if they are sick and should report COVID-19 test results, symptoms, and exposure via the Qualtrics System: UCR Symptom Monitoring Survey UCR Employee COVID-19 Hotline at 1-844-827-6827.

Effective Monday, July 20, all UCR employees (defined as faculty, staff, student interns and student workers) who are coming to work in UCR campus facilities will be required to monitor themselves for symptoms of the coronavirus and will be required to receive a clearance certificate from the Qualtrics System: UCR Symptom Monitoring Survey before beginning work each day on which they will be physically in the workplace. UCR will keep information about individuals’ symptoms confidential, accessible only to those with a need to know in order to address the situation. The information will not be maintained in personnel files.

Please note:  All employees who are working in campus facilities are required to complete the survey. Employees working remotely part of the time are required to complete the survey only on all days when they will be working in campus facilities.  Employees fully working remotely and who have no physical contact with co-workers may choose to complete the survey, but are not required to do so.

All employees who have internet access will receive a reminder email each Monday with a link to the Qualtrics System: UCR Symptom Monitoring Survey.  Each day, employees will go to this link to monitor themselves prior to reporting to campus by answering a few questions about their health status.  The survey may be completed on a smartphone, tablet or computer, and it should take no more than five minutes.

Employees who are unable to take the survey remotely will be required to complete it on campus before beginning work each day. Check with your supervisor regarding information about how this will be handled for your department. Upon completion of the survey, the system will either send an email with a clearance certificate or direct you to a follow up e-mail for further instructions. Copies of the clearance certificate will also be provided to your supervisor.
